We work with people experiencing disability as a result of a significant mental health issue, for which there is treatment provided by a suitably qualified practitioner. Synergy Wellness is a mental health needs assessment and service co-ordination service (NASC). There is information we need, such as details of the diagnosis and treatment, and the effect of illness symptoms on daily life, and this information is generally held by clinical services.
